I hereby certify that I have not knowingly withheld any information that might adversely affect my chances for employment and that the answers given by me are true and correct to the best of my knowledge. I further certify that I have personally completed this application. I understand that any omission or misstatement of material fact on this application or on any document used to secure employment shall be grounds for rejection of this application, or for immediate discharge if I am employed, regardless of the time elapsed before discovery. I hereby authorize any employers that I choose to investigate my references, work record, education, and credit history; to perform a criminal and social security number trace; and to check other matters related to my suitability for employment . I further authorize my former employers to disclose to the company any and all letters, reports and other information related to my work records without giving me prior notice of such disclosure. In addition, I hereby release Plug App and any of its affiliates or agents, my former employers, and all persons, corporations, partnerships, and associations from any and all claims, demands, or liabilities arising out of, or in any way related to, such investigation or disclosure. I also understand (1) some employers have a Drug and Alcohol Policy that provides for pre- and post-employment testing; (2) consent to and compliance with such Policy is a condition of my employment; and (3) continued employment is based on the successful passing of testing under such Policy. Plug App and its affiliates may require an associate to submit to a drug and/or alcohol test in the event of work related accidents, injury, or other circumstances. Failure to submit to or pass a drug or alcohol test will be grounds for immediate dismissal. I hereby acknowledge that any employment relationship I may enter into with Plug App or its affiliates is of an “at will” nature, which means that during the course of employment, I am free to end my employment with the company at any time, for any reason, and Plug App or any of its affiliates and I have the right to terminate employment at any time, with or without advance notice and with or without cause.
